Experience of Healthcare Workers Taking Postexposure Prophylaxis After Occupational HIV Exposures: Findings of the HIV Postexposure Prophylaxis Registry

Objective:
To collect information about the safety of taking antiretroviral drugs for human immunodeficiency virus (HIV) postexposure prophylaxis (PEP).
Design:A voluntary, confidential registry.
Setting:Hospital occupational health clinics, emergency departments, private physician offices, and health departments in the United States.
Results:492 healthcare workers (HCWs) who had occupational exposures to HIV, were prescribed HIV PEP, and agreed to be enrolled in the registry by their healthcare providers were prospectively enrolled in the registry. Three hundred eight (63%) of 492 of the PEP regimens prescribed for these HCWs consisted of at least three antiretroviral agents. Of the 449 HCWs for whom 6-week follow-up was available, 195 (43%) completed the PEP regimen as initially prescribed. Forty-four percent (n=197) of HCWs discontinued all PEP drugs and did not complete a PEP regimen. Thirteen percent (n=57) discontinued ≥1 drug or modified drug dosage or added a drug but did complete a course of PEP. Among the 254 HCWs who modified or discontinued the PEP regimen, the two most common reasons for doing so were because of adverse effects attributed to PEP (54%) and because the source-patient turned out to be HIV-negative (38%). Overall, 340 (76%) HCWs with 6-week follow-up reported some symptoms while on PEP: nausea (57%), fatigue or malaise (38%), headache (18%), vomiting (16%), diarrhea (14%), and myalgias or arthralgias (6%). The median time from start of PEP to onset of each of the five most frequently reported symptoms was 3 to 4 days. Only 37 (8%) HCWs with 6-week follow-up were reported to have laboratory abnormalities; review of the reported abnormalities revealed that most were unremarkable. Serious adverse events were reported to the registry for 6 HCWs; all but one event resolved by the 6-month follow-up visit. Fewer side effects were reported by HCWs taking two-drug PEP regimens than by HCWs taking three-drug PEP regimens.
Conclusions:Side effects from HIV PEP were very common but were rarely severe or serious. The nature and frequency of HIV PEP toxicity were consistent with information already available on the use of these antiretroviral agents. Clinicians prescribing HIV PEP need to counsel HCWs about PEP side effects and should know how to manage PEP toxicity when it arises.
Mesotaxy Layers of IrSi3 in (111)Si Formed by MeV ION Implantation

Iridium has been implanted at 1MeV and 500keV in the dose range 1 to 3×1017/cm2 at 525°C to form a buried Ir-rich region ∼ 2500Å below the surface in (111)Si. Samples were annealed at temperatures in the range 600°C to 1200°C to form buried IrSi3 layers. Rutherford backscattering and channeling analysis in conjunction with transmission electron microscopy were used to study the formation and the alignment of the IrSi3 layer to the silicon host as a function of annealing. For a dose of 2×1017 Ir/cm2, stoichiometric IrSi3 formed beneath ∼ 2000Å of Si after annealing to 1100°C. The IrSi3 layer was aligned with the Si substrate with Xmin ∼12%, but a band of precipitates remained either side of the main layer.
Lateral Confinement of Silicide Layers Synthesized with High Dose Implantation and Annealing

Using mesotaxy, a technique which involves high dose implantation followed by high temperature annealing, we have created narrow wires of CoSi2 buried beneath the surface of a silicon wafer. The implantation masks are fabricated directly on the silicon substrate using high resolution electron beam lithography in combination with reactive ion etching. TEM analysis shows that the wires are single-crystal and oriented with the substrate with very abrupt interfaces. The electrical continuity of the wires has been confirmed with electron-beam-induced current measurements.
Reconstruction of (100) Silicon/Disilicide Interfaces

A 2×1 reconstruction has been observed at the Si/NiSi2(100) and Si/CoSi2(100) interfaces. The reconstruction has been found in both ion-implanted (mesotaxial) material and in material grown by molecular beam epitaxy (MBE). The reconstruction is apparent in HREM images obtained from <110> cross sections and in transmission electron diffraction (TED) patterns from (100) orientation samples. We propose that the reconstruction is due to a layer of dimerised silicon atoms at the interface. We conclude that the 2×1 reconstruction is a low energy state of the silicon/disilicide(100) interface.
Ion-Beam-Induced Destruction of Superconducting Phase Coherence in YBa2Cu3O7-δ

We have explored the effects of ion beam irradiation on the electrical and structural properties of superconducting thin films of YBa2Cu3O7-δ. Damage created by nuclear energy loss processes degrades the superconducting transitions. In general, the onset temperatures do not vary significantly with ion fluence, but the transition widths broaden until the resistance no longer reaches zero. This behavior is strikingly similar to that observed in granular superconducting films where islands of superconductor are progressively decoupled from each other, destroying the phase coherence. Data from resistance measurements as well as from Rutherford Backscattering and channeling measurements suggest that ion bombardment is not changing the volume fraction of superconducting material substantially.

The microstructure of oxygen implanted silicon for use in silicon-on- insulator technology has been examined by transmission electron microscopy. A variety of buried oxide layers prepared using oxygen doses below and above that required for stoichiometric SiO2 formation have been studied. High resolution imaging in crosssection has revealed exceptionally flat Si-SiO2 interfaces, comparable to the best thermally grown Si-SiO2 interfaces. Examination of as-implanted material shows a complex interwoven crystalline/amorphous structure which evolves during high temperature (1350–1400° C) annealing into a buried oxide layer.
